Making an enquiry
When enquiring about Trusted Weight Loss services, either via online enquiry form or direct telephone communication we will generally collect data in the form of your name, email and telephone number. This data is collected to ensure we send any appropriate information to the correct location.
Booking a consultation
When making an appointment for a consultation Trusted Weight Loss will collect further personal and medical information from you in order to assess your suitability for the services we offer. Information collected may include, but is not limited to, your Date of Birth, Height, Weight, Medical history. This information is collected and shared securely with your consulting Doctor so that he/she may make an accurate assessment of your eligibility for treatment.
By providing your personal data, required for booking a consultation, you are consenting to our Privacy Policy and the secure sharing of this data with the consulting Doctor. Data will be maintained for a 2-year period following the last communication with Trusted Weight Loss or for as long as is required for legal or insurance purposes. You can request that we delete the data we hold on you at any time.
Booking Allurion Balloon Treatment
Should you be eligible for treatment and wish to proceed the Trusted Weight Loss team will book your treatment with the chosen Doctor and Clinic. When you decide to pay your deposit and book treatment you are entering a contract with Trusted Weight Loss under which Trusted Weight Loss will supply services. Under the Terms and Conditions, Trusted Weight Loss’s Data Protection policy will apply.
The information you have already supplied, along with consultation notes and any other note for your safe treatment will be securely shared with the chosen Clinic and Doctor.
If any additional test or investigations are needed to confirm your eligibility for treatment we will share certain data with third party suppliers to carry these out. Your consent will be requested beforehand. Only information necessary to undertake your treatment safely will be shared.
Trusted Weight Loss will write to your GP to inform them of your treatment as we feel this is pertinent to the ongoing management of your general health. We will also provide a discharge letter confirming the treatment and initial medication advice. You can request us not to inform your GP.
- Children
Trusted Weight Loss treatment, website and advice is not intended for children and we do not knowingly collect data relating to children. The minimum age limit for undergoing treatment is at least 18 years of age. If we receive enquiries from children, we will direct them to their parent/guardian or GP.
- Data security
We have put in place appropriate security measures and take reasonable precautions to prevent your personal data from being accidentally lost, used or accessed in an unauthorised way, altered or disclosed. We will store all personal information you provide on our secure servers.
We have put in place procedures to deal with any suspected personal data breach and a Data Breach Investigation Policy and we will notify you and any applicable regulator of a breach where we are legally required to do so.
